Abstract Background and Aims Antibody-mediated rejection (ABMR) is a frequent cause of renal allograft loss. There increasing evidence the role Natural Killer (NK) cells in establishment ABMR damage. Our group described that patients with donor-specific antibodies (DSA) present higher proportions NKG2A+ NK cell subset peripheral blood than those without HLA DSA or antibodies. Method We selected 177 kidney transplant recipients (KT) biopsies 2011-2017: 77 (DSA+: 53, DSA-: 24) 100 15, 85). assessed graft survival median time follow-up since biopsy 53 months. In 138 KT we evaluated immunophenotyping its value as prognostic biomarker. Results Graft was worse ABMR-KT at end (p<0.001) independently detection (p=0.63). Regarding immunophenotyping, observed lower proportion absolute count ABMR+DSA+-KT ABMR+DSA--KT compared ABMR-DSA--KT (p=0.027, p=0.017). showed (p=0.007). All ABMR+ patients, detection, presented NKG2A- comparison (p=0.001, Finally, <30% associated 36 months after (p=0.067) (Figure). Conclusion ABMR- detection. Kidney show reduced numbers regardless undetectable DSA. This phenotype medium-term cases ABMR.
